For manual submission of Nominations the following is needed:

Department, Degree and Year earned
Complete Home Address
Business Address (if applicable for communications)
Your email address
Your cellphone & other pertinent phone contact numbers

Current Occupation of the Nominee

Brief Summary of Nominee’s Accomplishments with special attention to the impact of their work. You may submit supporting documentation which may include: CV (short version) or Resume’, Letters of Recommendation, Newspaper/magazine reprints, description of other awards/honors or a brief biography and pictures to: GHKawards@gmail.com.

If an electronic form, of any supporting document, is not electronically available, please mail hard copies to:
